Birmingham Children’s Hospital now offers three one year posts in paediatric anaesthesia. They aim to provide training to advanced level in paediatric anaesthesia as outlined in the Royal College of Anaesthetists 2010 curriculum for the CCT in anaesthesia.

These posts are suitable for trainees during or after their higher level training in anaesthesia – ie in years 5-7 of Speciality Training, or for those who have recently acquired the CCT in anaesthesia and wish to spend a period of consolidation in the subspeciality. It is expected that fellows will have already done a higher level placement in paediatric anaesthesia. Two of the posts start in May and are usually advertised the previous July / August with interviews in September / October. One post will start in November and will be advertised in February / March of that year.

The hospital can provide excellent clinical experience in all areas of paediatric surgery and anaesthesia, including cardiac, cardiology, hepatobiliary, liver and small bowl transplantation, general surgery including laparoscopic surgery, burns and reconstructive surgery, plastic surgery including cleft lip and palate surgery, orthopaedic surgery, ENT, urology, ophthalmology including retinoblastoma treatment, craniofacial surgery, neurosurgery, maxillofacial surgery, oral and dental surgery. In addition, the department provides anaesthesia for: paediatric endoscopies, laser treatment for haemangiomas, joint injections for children with arthritis, paediatric radiotherapy, MRI, CT, Interventional Radiology, oncology treatment and investigation including intrathecal chemotherapy, bone marrow aspirate and trephine, insertion of hickmann lines. There is an active acute pain service run by a combination of anaesthetic consultants and acute pain nurses. The department currently has 27 consultant anaesthetists and separate consultant on-call rotas for general, cardiac and liver duties.

The anaesthetic service at Birmingham Children’s Hospital is largely consultant based, however there are ample opportunities for fellows to do solo lists to gain confidence and experience. As with the Speciality Trainees, the Fellows will always work under supervision of a nominated consultant anaesthetist at all times. The level of supervision for fellows is identical to speciality trainees – ie all will have a minimum of three directly supervised sessions per week on average. The educational opportunities are also the same as for speciality trainees, and fellows are encouraged to attend weekly tutorials and department meetings – typically 2-3 hours per week. Fellows are encouraged to be involved in audit and research, time will be provided to do this to reflect the needs of the individual – usually one day per week. Currently the fellows take part in the Speciality Trainee resident on-call rota.

The hospital has a very busy 22 bedded PICU, which is being expanded to 30 beds over the next few years. Most fellows in paediatric anaesthesia will spend 2-3 months working in PICU during the one year placement.

Our fellowship is based in the Bristol Royal Hospital for Children (BRHC), part of the University Hospitals Bristol and WestonNHS Foundation Trust (UHBW).

The paediatric anaesthesia fellowship is available for post CCT (or equivalent) doctors.

The post usually runs for 12 months, starting in August. Shorter fellowships may be available from time to time.

BRHC is the Regional Children’s Hospital for the South West and moved to its current site in 2001. It contains the regional PICU & retrieval service.  The Anaesthetic department consists of 24 Consultant Paediatric anaesthetists, 1 Specialist Doctor and trainees from the Bristol or the Southwest School of Anaesthesia

The Royal Hospital for Children (formerly known as “Yorkhill”) is the largest paediatric hospital in Scotland. We moved to our new site in June of 2015, becoming part of one of the largest and busiest hospital campuses in Europe. Our 244 bed tertiary referral centre serves the West of Scotland and beyond, for acute and elective surgery for children up to age 16, and administers  approximately 16000 anaesthetics annually. The hospital is the Scottish Major Trauma centre for paediatrics.  All surgical specialties are accommodated here, and the hospital holds multiple national service contracts: Cardiac surgery and interventional cardiology; Renal transplantation; Cleft palate surgery; Chest wall surgery; Complex paediatric airway surgery (including laryngo-tracheal reconstruction and laser surgery); craniofacial and neurosurgery.  (Spinal scoliosis correction surgery is performed at Edinburgh Sick Children’s hospital, access to which is accommodated during the fellowship).

The hospital is physically linked to the maternity unit and Neonatal Intensive Care Unit and has a large complex neonatal workload. Fellows can expect regular exposure to neonatal anaesthesia, with many surgical procedures carried out in NICU itself. The theatre workload is supported by a 22 bed critical care unit (PICU) which is staffed by physician and anaesthetic intensivists, and provides ECLS support. Many of the staff also have commitments to ScotStar, the national paediatric retrieval service. The theatre suite comprises of 9 theatres and accommodates the following: dedicated emergency theatre, trauma, general surgery (including thoracosopic), urology, orthopaedics, ENT, plastics, ophthalmology, neurosurgery, cranio facial and maxillo-facial surgery, dentistry, oncology. Within the theatre suite we have interventional radiology and cardiac catheterization suites, and are co-located with MRI, CT scanning and nuclear imaging.  In addition the department provides anaesthesia services off-site e.g. radiotherapy at the purpose built oncology hospital. The department has a comprehensive consultant led acute and chronic pain service with 3 full time specialist pain nurses and multidisciplinary team support. There is an active rolling teaching programme delivered by consultants and supported by a supplementary teaching programme run by PICU. Monthly CME meetings comprise of educational components, Morbidity and Mortality meetings, and QI updates. Fellows would have the opportunity to present at these meetings, and would also have a commitment to organising and delivering teaching to other anaesthetic trainees and other members of the theatre team. The department has a strong quality improvement/audit programme and is active in large international research projects. Research ideas that fellows may have can be facilitated. The department would encourage fellows to participate in such projects and could contribute funding for attendance at national and international meetings to facilitate presentations of such projects. The post- CCT fellowship positions are open to anyone holding a CCT in anaesthesia, and meet the RCOA criteria of completing higher level paediatric training or equivalent. The fellowships can be 6-12 months in duration. 12 month posts will comprises a total of 9 months of paediatric anaesthesia with a 3 month rotation to PICU. The post can be tailored according to training requirements and special interests. These posts are appropriate for individuals aiming to be tertiary paediatric anaesthetists, or for those wishing to have a paediatric interest in a non-tertiary centre.  The year will give multiple opportunities to develop both professionally and personally, in preparation for taking on the consultant role.

About the Fellowship:

We offer two one-year posts in advanced training for paediatric anaesthesia. They provide advanced level training in paediatric anaesthesia as outlined in the Royal College of Anaesthetists 2010 curriculum for the CCT in anaesthesia. These posts are suitable for trainees during their final years of Specialty Training or for those who have recently acquired the CCT in anaesthesia.

 

Leeds is one of the largest Children’s Hospitals in the UK with 286 beds and we deliver over 10000 surgical procedures to children every year, including over 300 neonatal procedures. These fellowships provide excellent clinical experience and follow a structured training scheme encompassing all the sub-specialties that Leeds Children’s Hospital has to offer. As well as serving the local population, we are a large tertiary referral centre for complex Neonatal and Paediatric surgery including Congenital Cardiac, Neurosurgery, Craniofacial, Thoracics, Urology, Haematology and Oncology, Maxillo-facial, Major Paediatric Trauma and Orthopaedics. Our supraregional services include a Paediatric Transplantation programme for livers, kidneys and bone marrow. We are one of the largest plastic microsurgery and the largest children’s hand reconstructive unit in the UK. We were the first paediatric centre in the UK to offer robotic surgery. We are always striving for success and building is currently underway for our new Hybrid Lab and intraoperative 3T MRI.

 

We are a friendly, supportive department of around 25 Consultant Paediatric Anaesthetists with excellent feedback from previous fellows. We have 8 dedicated Paediatric theatres where we provide anaesthesia for all the surgical sub-specialties as well as medical specialties including Gastroenterology, Rheumatology and Dermatology. We also provide anaesthesia outside the theatre environment for Radiotherapy, MRI, CT, Interventional Radiology and Oncology treatment.

 

We have an Integrated pain service led by anaesthetic consultants and specialist paediatric pain nurses.

 

The hospital has a 6-bedded HDU and 16 bedded PICU. The fellowship includes a 3-month placement in PICU as part of the sub-specialty rotation. There is also a large tertiary NICU within the Children’s Hospital.

 

Fellows are encouraged to attend the regional Paediatric anaesthesia teaching, in house MEPA course and bimonthly journal club. Ample opportunities exist, to develop teaching skills and to take part in audit, quality improvement and research. Half a day per week is provided for this. The clinical fellows take part in the Specialty Trainee resident on-call rota, which is currently a 1 in 8.

Alder Hey Children’s Hospital is one of Europe’s busiest paediatric hospitals and we have recently moved to our new facility in Alder Hey in the Park with 270 acute beds, 48 of which are for critical care patients. The hospital is a regional and supraregional referral centre for craniofacial surgery, children’s epilepsy surgery and cardiac surgery including ECMO. Experience can be gained in these specialties and also major orthopaedic, neurosurgery, neonatal, general, urology, spinal surgery, oncology, ENT and cleft palate surgery, cardiac catheterisation, interventional radiology/CT/MRI, endoscopies, ophthalmology, maxillo-facial/dental, plastics and burns. Approximately 22,000 patients undergo general anaesthesia each year within the 12 inpatient theatres, 5 day case theatres and MRI and radiology suites. The department provides an acute and multidisciplinary chronic pain service. There are currently 35 consultant anaesthetists , 14 rotating specialty trainees from the Mersey deanery and 6 fellows.

The posts are normally occupied by anaesthetists who already hold FRCA or equivalent in the final years of specialist training (ST 5,6,7) or post-CCT. 2 of the posts are normally allocated to applicants from overseas and subject to the necessary GMC language and other requirements being met, we are able to provide sponsorship for GMC registration and Tier 2 Visa for successful applicants. The posts are for one year and have educational approval for training from the Postgraduate Dean and Royal College of Anaesthetists for up to 12 months of Advanced Anaesthesia. The post-holder will spend 9 months of the fellowship working directly in anaesthesia, the remaining time divided into a 6 week block in intensive care and a further 6 weeks working with the North West Paediatric Transport Service (NWTS) stabilising and transferring critically ill children. Fellows undertake 1st on call duties on the Specialist Registrar rota during the week and at the weekend form a separate higher fellow rota to enable greater opportunity for emergency management of neonatal cases. We are also able to offer a specific cardiac fellowship focused on cardiac theatre and interventional catheter procedures however this would be offered based on the suitability of applicants at the time of interview.

Research and quality improvement projects are actively encouraged and co-ordinated by Dr Phil Arnold; where appropriate time will be allowed on the weekly rota for research (up to a maximum of half a day per week.). The department holds monthly ½ day meetings which includes audit, journal

reviews and presentations on topic of general interest. There is a strong interest in ultrasound guided regional anaesthesia within the department for all age groups; we run an ESRA credited 2 day course and teaching on regional anaesthesia is normally provided one morning a week. We also hold Paediatric Airway Workshops giving practical teaching on advanced airway techniques and one lung ventilation techniques for neonates and older children. In addition to the above there is once weekly teaching on core paediatric topics.

The anaesthetic service is largely consultant based however several lists will be run independently by trainees with a Duty Consultant available to provide advice, practical support and teaching on the theatre lists. Fellows should by the end of the year have not only developed their practical skills but also confidence and decision making ready to take up a consultant post in a tertiary paediatric centre.

Great Ormond Street Hospital (GOSH) currently offers eight one year posts and two eighteen month post in paediatric anaesthesia that are advertised in open competition. These are in addition to other one year and six month posts that are appointed from the London Schools of Anaesthesia. The posts are all Trust funded and are not officially recognised as training posts but prospective approval for training by the College has never been a problem. They all aim to provide training to an advanced level in paediatric anaesthesia as outlined in the Royal College of Anaesthetists 2010 curriculum for the CCT in anaesthesia.

The posts are aimed at trainees who are near the end of their speciality training in anaesthesia – ie in years ST 5-7, or at those who have recently acquired the CCT in anaesthesia and wish to spend a period of consolidation in this subspecialty. All applicants should be able to demonstrate a commitment to a career in paediatric anaesthesia in their application and have completed higher paediatric anaesthetic training. Some posts are specifically aimed at those who have already done some advanced training in the speciality. The posts start at different times of the year but stick to the usual click dates of February, May, August and November. We tend to advertise 6-12 months in advance of the start date for each post and the adverts appear in the NHS Jobs and the GOSH websites.

GOSH is a tertiary referral hospital with are more than 50 different clinical specialities. We provide anaesthetic services for all surgical, advanced imaging and treatment specialties within the hospital. Due to the complex nature of both the surgery we offer and the children we treat paediatric anaesthesia is a consultant led service and fellows should expect a high level of direct supervision of their work, especially in the first few months of their time at GOSH. With time and with appropriate supervision by consultants, fellows will be allowed to assume the role of lead clinician for elective lists and emergency work.

There are plenty of educational opportunities on offer for all trainees. We have weekly departmental meetings, two weekly protected teaching sessions (2-3 hours) and the opportunity to attend the active teaching program laid on around the Trust. Trainees are expected to take an active role in Audit, Research and Training whilst at GOSH. There are many opportunities in each of these areas.

About the fellowships

Fellow in paediatric and congenital cardiac anaesthesia

The Freeman Hospital is unique in that it is the only hospital in the country where, under one roof, any patient of any age with congenital heart disease (CHD) may be seen, assessed and treated, and potentially offered all available therapies for CHD including mechanical support and transplantation. The fellowship programme - in paediatric/congenital cardiothoracic anaesthesia - therefore offers a comprehensive training programme in all aspects of the anaesthetic/perioperative care of the child with CHD (including hypoplastic left heart syndrome), together with mechanical support (ECMO and ventricular assist device) and cardiopulmonary transplantation. You will also gain valuable experience in anaesthesia for paediatric thoracic surgery, paediatric cardiac intensive care and the perioperative care of the adult with CHD.

Fellows are allocated to the operating theatres, the catheterisation laboratory, the intensive care unit (for ‘on call’ duties) or occasionally other lists covered by the consultant paediatric cardiac anaesthetists. Initial induction training includes some structured tutorials on some of the more common congenital lesions, and fellows receive educational support and advice during the year. By the end of the year, most trainees would expect to complete a more straightforward infant case with distant consultant supervision. They would also expect to achieve competence in the placement of invasive intravascular lines, the use of ultrasound, airway management and interventions, and the support of the circulation with vasoactive drugs.

Fellows are typically appointed towards the end of their period as a trainee. Generally we look to appoint someone with some experience of general paediatric anaesthesia, adult cardiac anaesthesia, paediatric intensive care and paediatric cardiac anaesthesia, although we are not unduly proscriptive about this. The majority of fellows are contemplating a career as a consultant paediatric cardiac anaesthetist, and the majority will go on to an appointment as such after completing the fellowship. Some fellows develop particular interests during their year, including research, audit and teaching; we encourage appropriate use of study leave. There are many weekly departmental meetings to attend. Those trainees who have an interest in TOE are encouraged, subject to making appropriate progress in anaesthetic management of this highly complex and challenging group of patients.

About the fellowships:

Our fellowship is aimed at those wishing to spend 12 months in a children’s hospital in pursuit of specialist paediatric experience. Applicants should hold the CCT in Anaesthetics; OR Registration on GMC Specialist Register in Anaesthetics; OR within six months of achieving CCT at the time of interview.

During a 1 year post at Children’s Hospital Oxford (CHOX) the anaesthetic trainee will have the opportunity to experience all subspecialties of paediatric anaesthesia (except cardiac anaesthesia-experience can be organised externally if requested) and may undertake a 3 month placement in the paediatric intensive care unit (ICU) if required. Anaesthetic training is based on a modular system and includes time spent doing blocks of neuro, craniofacial, ear nose and throat (ENT), plastics and general surgery.

Training modules and competencies are all based around the RCoA Certificate of Completion of Specialist Training (CCST) guide and CHOX fulfils all their criteria for higher specialist training in Paediatric Anaesthesia. There is also a comprehensive appraisal/educational support process in place.

Elective surgical specialties include general and urological surgery, orthopaedics, craniofacial, plastics, neurosurgery, ENT, dental and ophthalmology whilst the medical specialities include oncology, radiology (including MRI), radiotherapy, gastroenterology and cardiology. There is a consultant led anaesthetic service for neonatal emergencies (currently around 200 cases per year). There are currently 11 Consultant Paediatric Anaesthetists. Within the group there are on-going audit and research projects.

Sheffield Children’s Hospital is a long established, stand alone, specialist children’s hospital, anaesthetising about 16,000 cases each year. It takes secondary and tertiary referrals from South Yorkshire and beyond. Geographically it is ideally situated, being very close to the University, adult teaching hospitals and the Peak District National Park.

The Anaesthetic Dept has 22 full-time specialist Paediatric Consultant Anaesthetists, and 7-8 rotating specialist registrars, one of whom will be doing an identified extended specialist training slot. The theatre complex has 7 theatres and a procedure room with building underway for two new theatres, one with an integrated 3T MRI facility.

This Fellowship is for one year, ideally post CCT or as an OOPE. Experience will be gained in specialist general surgery, orthopaedics including spines, ENT including laryngeal reconstruction, neuro-surgery including stereotactic, plastics, max-fax, ophthalmology, burns, gastroenterology, dental, medical lists, oncology, MRI and acute pain.

There is a wealth of experience in ultrasound guided regional bocks using catheters. There will be the opportunity for an attachment to the 9 bedded PICU/8 bedded HDU which has 7 dedicated PICU Consultants. There will be the opportunity for experience in chronic pain as SCH has an established multi-disciplinary pain service lead by Anaesthesia.

Embrace (the regional paediatric transfer service) is run from our Directorate, so experience can be arranged in this area if requested. Should the candidate wish to get exposure to paediatric cardiac anaesthesia, this can be organised as a secondment within the rotation.

The Fellow will receive in-theatre and tutorial training along with the rotating registrars and will be given the opportunity to attend specialist paediatric anaesthesia trainee meetings,  co-ordinated by the specialist centres in the north of England.

The department has a strong commitment to teaching with many Consultants being APLS and MEPA Instructors The successful candidate will take part in the resident SpR on-call rota.

We are the largest and busiest children’s hospital in the UK with 371 paediatric beds. A further 60 neonatal cots also operate, within the same building, in the tertiary neonatal unit of Saint Mary’s Hospital.

The single site Royal Manchester Children’s Hospital (RMCH) opened in June 2009. We provide general paediatric services for the local population and tertiary paediatric services for a large part of the North West of England and beyond. The hospital is situated on the south-eastern corner of the Manchester University Foundation Trust (MFT) site and benefits from easy access to and support from specialists such as ophthalmology, obstetrics & vascular surgery who are co-sited in the adult divisions of the trust.

RMCH is recognised as a Children’s Major Trauma Centre for the North West seeing approximately 150 ISS>9 patients each year. Full laboratory and both diagnostic and interventional radiological support is provided. MR and CT scanners are available 24 hours a day as . RMCH plays an important role in medical teaching. It is the major paediatric teaching hospital for the University of Manchester.

THE POST – CLINICAL FELLOW IN PAEDIATRIC ANAESTHESIA 

 

DUTIES OF THE POST

Duties will be based at the Royal Manchester Children’s Hospitals as a member of the Anaesthesia Team.  The successful candidate will be exposed to the full range of specialities available at the children’s hospital. During this time you should try and see a wide range of children undergoing multiple different procedures. The job of an anaesthetist is to provide peri-operative care to patients in the safest way possible. Individual patient care is required with a family centred approach. This includes pre-operative assessment, peri-operative anaesthetising and post-operative pain management strategies.An opportunity to work on PICU and/or NWTS may be available by mutual agreement. 

As a clinical fellow in paediatric anaesthesia there is flexibility to ensure both your learning needs and the service provision. Educational supervision and support will be provided using face-to-face teaching via a named educational supervisor who will help you determine your learning agreement for your placement. You will also have access to trained mentors and expected to maintain the new RCoA e-portfolio for revalidation and career progression as well as the Trust appraisal process. 

PERIOD OF TIME OF POST

This post is for an initial period of six months with possible extension to twelve months.  

STAFFING -ANAESTHETISTS

The department consists of 36 anaesthetic consultants. 

The department provides anaesthesia and acute pain management services for the full range of specialities offered at the Children’s Hospital. 

To enable the new arrangements for higher specialist paediatric anaesthetic training, all anaesthetic trainees within the Manchester HEENW rotate through RMCH for at least 3 months. There are 16 trainees for each 3 months. 

Anaesthesia is provided for children both within the Children’s theatres and at other sites within the trust, including the Burns Unit theatre, the Eye Hospital, X-ray department, and Oncology clinic. The department also provides services for paediatric patients at Trafford General Hospital and the radiotherapy department at the Christies Hospital, with future developments of providing anaesthetic care for Proton Beam therapy starting August 2018.

There is a paediatric anaesthetic consultant on call 24 hours per day, and there are daytime CEPOD, dedicated Neonatal CEPOD lists as well as Trauma and Burns lists.

DEPARTMENTAL WORKLOAD

The department undertakes approximately 11,000 anaesthetics per year. There are approximately 750 intensive care admissions per year.  

TEACHING

There are once weekly Anaesthetic Department teaching sessions. There are also bi-monthly joint education days with Alder Hey and other children’s Hospitals. 

CONTINUING MEDICAL EDUCATION

In 2017 the department launched a MSc in Paediatric Anaesthesia (3 year part time course) with Manchester University. This is the first MSc in paediatric anaesthesia in the world. During the year placement dependent on funding it would be possible for a successful candidate to work towards credits for the MSc.

AUDIT 

The appointee will be expected to participate in audits and the development of protocols and care pathways specific to the service. Clinical Fellows are expected to be aware of the principles of clinical governance and to contribute to the aims of Paediatric Anaesthetic Department and the achievement of ACSA accreditation. 

Any research interests can be discussed and agreed on a case-by-case basis.

One Fellowship position in Paediatric Anaesthesia is available, for a period of 12 months. The post is designed for those wishing to pursue a career as a specialist paediatric anaesthetist, and fulfils all the criteria described in the Royal College of Anaesthetist 2010 curriculum. Applicants may be pre or post CCT, must hold FRCA or equivalent, and should have already completed their higher level training in paediatric anaesthesia. 

All paediatric specialities within Nottingham University Hospital (NUH) are located at the Queens Medical Centre campus. Nottingham Children’s Hospital has 116 acute beds and a stand-alone Paediatric Emergency Department, forming part of the Nottingham Major Trauma Centre. The 10 bedded Paediatric Intensive Care Unit specialises in management of major trauma, neurosurgery, renal disease, spinal surgery and oncology. The department also has an active Paediatric Acute Pain Service, led by Dr H King working with 2 Clinical Nurse Specialists. The service sees approximately 1000 patients per year.

Approximately 10,000 paediatric surgical procedures are carried out each year at NUH, including emergency work, in 7 operating theatres across 3 theatre suites and 3 satellite areas. There is a significant neonatal surgery workload and NUH is a tertiary referral centre for neurosurgery, paediatric oncology and radiotherapy, paediatric urology and renal transplantation, spinal surgery and cleft lip and palate surgery. There is also a large ENT, emergency, general and plastic surgery workload. The NUH Major Trauma Centre is one of the busiest in the country, with a significant number of paediatric major trauma cases.

Currently, there are 14 full-time paediatric anaesthetists, who provide anaesthesia for most paediatric sub specialities, with the exception of paediatric cardiac and craniofacial surgery. The anaesthetic department has close links with both the Paediatric Intensive Care Unit and Neonatal intensive Care Unit. Training time can be arranged in either of these units depending on the development needs of the successful candidate.

The anaesthetic department has an active teaching program, comprising morbidity and mortality, audit, journal reviews and training in subspeciality areas of anaesthesia. The fellow will be expected to contribute regularly to these meetings. There are also excellent research links with the University Department of Anaesthesia, Nottingham University and Nottingham Trent University. Active research projects are always ongoing, and there is opportunity for the appointee to develop their own ideas.

One day a week will be free for research or audit activities and there will be opportunities to gain experience in PICU and cardiac anaesthesia in another local centre.

Temple Street Children’s University Hospital was founded in 1872, and provides care for 148,000 per year.  Recently renamed CHI at Temple Street, it continues as a 100 bed tertiary referral paediatric hospital in north Dublin, Ireland.  We anaesthetize over 7500 children per year, between 0 and 16yrs for multiple medical and surgical specialties including neonatal and congential surgeries, as well as being the national centre for metabolic diseases, paediatric renal transplant, and paediatric neurology and neurosurgery.  Additional specialties include, but are not limited to:

The one-year fellow will be expected to participate in and be exposed to all the surgical specialties, and will be given appropriate non-clinical time in order to pursue research, audit and quality improvement.  Involvement in PICU and retrieval services can be organized depending on individual preference. This year will give ample opportunity to develop professionally, clinically and personally towards a consultant role either as a Paediatric Anaesthesiologist, or General Anaesthesiologist with a special interest.

CHI at Temple Street is part of an ongoing transformational organization, whereby Ireland’s three paediatric hospitals (Temple Street, Our Lady’s Crumlin and Tallaght) will amalgamate to the new centralized CHI hospital site - with 22 operating theatres - opening in 2023, alongside additional external paediatric outpatient and urgent care centres around the city.  In order to meet this increased service, we foresee good opportunities for paediatric anaesthesiology consultant posts in the future.

About the fellowship: 

Perth Children’s Hospital, formerly Princess Margaret Hospital, is the sole tertiary paediatric hospital for Western Australia.  The hospital moved to a purpose built, modern facility that opened in 2018.  PCH has 298 beds, 18 PICU/HDU beds and 30 NICU beds. PCH services a population of approximately 2.5 million people.

The department of anaesthesia has 37 consultant staff, 3 anaesthesia fellows and 8 registrars. We perform approximately 13,000 anaesthetics per year within 12 operating theatres, including hybrid theatres and an intraoperative MRI suite.

PCH offers a full range of paediatric surgical services, including neonatal surgery, cardiac surgery, neurosurgery, airway surgery (including airway reconstruction), craniofacial surgery, trauma surgery, scoliosis and multilevel orthopaedic surgery.  Heart, lung and liver transplantation are not performed at PCH.  There is a well-established inpatient Acute Pain Service with twice daily rounds, a CVAD service and an outpatient multidisciplinary Chronic Pain Service.

Starship Child Health is a tertiary referral children’s hospital for the whole of New Zealand and surrounding Pacific Islands. Pathology is a mixture of first and third world medicine providing a very varied clinical experience. Starship is the sole provider of congenital cardiac surgery in New Zealand.

We offer all neonatal and paediatric sub-specialities other than burns and plastics, which are performed at a nearby hospital. The PICU is busy, with a high number of cardiac patients and retrievals from around the country.

The department has an active audit and research programme for those interested in getting involved with projects during the year.

About the fellowship

The Department of Pediatric Anesthesia at BC Children's Hospital is one of 11 similar university affiliated tertiary care programs in Canada. BC Children’s Hospital (BCCH) is the major tertiary care referral centre for British Columbia and the Yukon. Over 9000 surgical cases are performed each year. The fellowship is a mixed clinical and research position. About 60% of time is spent in clinical practice, and 40% in academic endeavor. There is a large Day Care Surgery Program. Anesthesia is provided for all aspects of pediatric surgery including cardiovascular and neurosurgery and for areas outside the operating room including radiology and the burn unit. There is a busy acute pain service and liaison with the complex pain management program. Children’s Hospital is affiliated with Women’s Health Centre, the tertiary care obstetrical hospital for British Columbia, which is the largest maternal‐fetal‐newborn clinical service in Canada, and hence there is considerable volume of newborn surgery. A busy Pediatric Intensive Care Unit with over 20 beds and a 50 Neonatal Intensive Care Unit are situated within the Hospital. Fellows complete an average of 600 cases of which over 50% are completed independently and have a case mix of 21-28% being under one year of age. A 4 week cardiac rotation provided experience with at least 20 OH cases. We have an active staff with special interests in acute and complex pain management, difficult airway management, ultrasound guided procedures, education, administration, trauma, spinal instrumentation, pediatric anesthesia in the developing world and other concentrations.

Fellows are expected to be academically productive during their time at Children’s. The importance of “pre-fellowship” preparation for project design, IRB approvals and funding is emphasized by early acceptance of applicants and assignment of staff research mentors to facilitate planning prior to the fellowship year. In addition upon acceptance, fellows are encouraged to prepare for their fellowship year by consolidating skills in basic software programs such as Word, Excel, Presentation software, Reference Managers and Mendeley.

About the fellowship:

Introduction:

The Pediatric Anesthesia Fellowship at the Hospital for Sick Children is a twelve-month education and training program in all aspects of pediatric anesthesia. The clinical and didactic education curriculum includes pediatric pain management and perioperative anesthetic care for neonates, infants, children and adolescents both in the operating room and in satellite locations.

Fellows are expected to participate in departmental educational activities and clinical and/or basic science research. Relevant to their level of training and experience, fellows also assume graduated independence and responsibility for supervision of residents assigned to the service.

Training objective:

To prepare a trainee who aspires to a career in full-time pediatric anesthesia practice in a specialist pediatric hospital or a tertiary referral centre. The level of proficiency that is expected at the end of the fellowship is in keeping with the CanMEDS (Canadian Medical Education Directives for Specialists) framework.

Curriculum

Clinical program:

Fellows work under staff supervision in the provision of anesthetic care in pediatric patients undergoing operations in all surgical subspecialties including cardiac, thoracic, abdominal, craniofacial, neurosurgical, ENT, orthopedic, urological, and transplant surgery. Experience will also be gained in the anesthetic management of patients in satellite locations undergoing diagnostic and non-operative procedures including diagnostic and interventional radiology, cardiac catheterization and endoscopy.

Fellows actively participate in the acute pain management of pediatric patients through the performance of neuraxial and peripheral nerve blockade in the operating room and regular rotations with the Acute Pain Service. Exposure to chronic pediatric pain management is gained by elective attendance of the Chronic Pain Clinic.

Education:

A lecture series and case-based discussions provide teaching of core topics in general pediatric anesthesia, pediatric cardiac anesthesia and pain management. Other educational activities include weekly departmental rounds, Telehealth conferences and simulation sessions.

Research:

Fellows are allocated one day a week of non-clinical time and are expected to participate in clinical and/or basic science research projects that are ongoing within the department. They are encouraged and supported to contribute to the design, data collection and analysis of a research project. Fellows are expected to attend a lecture series in research design and statistical methods. Submission of research findings to national and international anaesthesia meetings is encouraged and supported.

The Montreal Children’s Hospital is a teaching and research center located in the vibrant and beautiful city of Montreal. We are a tertiary-care centre with approximately 7000 pediatric cases per year.

The MCH is a designated pediatric trauma center and has a very busy emergency room, a Neonatal Intensive Care Unit, and a Pediatric Intensive Care Unit. The Main Operating Block has 7 operating rooms, with 2 sedation rooms. The Shriners Hospital for Children in Montreal is a specialized orthopedic institution founded in 1925. The hospital is a 40-bed facility with two operating rooms, and one pre-induction room. The hospital does about 800 surgeries every year; about half are performed as outpatient surgery. This includes routine and complex orthopedic surgery, scoliosis surgery, and urological surgeries. The hospital is also an orthopedic referral center for patients with osteogenesis imperfecta, offering specialized care for patients from the rest of Canada and internationally. The hospital is affiliated with the McGill University Health Center (MUHC). Medically complex patients, who may require admission to the intensive care unit, are treated at the Montreal Children’s Hospital.

The Children’s’ Hospital LHSC is a tertiary referral centre. There are over 8200 anesthetic cases each year at Children’s Hospital including neurosurgical, craniofacial, thoracic, neonatal, major orthopedic and urologic, general, ENT, and plastic surgical procedures. Several clinicians have interest and experience in the delivery of Pediatric regional anesthesia. In addition, we have an active Pediatric Chronic pain program and Transitional Pain service that has a weekly multidisciplinary clinic.

New technologies have increased the requirement for ‘off-site’ procedures and the anesthesia department at Children’s Hospital currently provides anesthesia for children in ten locations within the Health Sciences Center and University hospital. We also have a dedicated Pediatric Anesthesia Pre-admission Clinic.

All anesthesiologists within the Section have university appointments within the Department of Anesthesia and some are cross appointed with the Department of Pediatrics. Most staff hold pediatric anesthesia fellowships from major pediatric centers like Sick Kids, CHEO and Great Ormond street. 

Fellowship Structure

  • Duration: This will be a 13-block fellowship, including 4 weeks of vacation. The remaining 48 weeks will comprise the clinical and academic time as detailed in the following sections. The fellowship will be organized into four-week periods, in order to be compatible with the residency program scheduling structure.
  • Number of fellowship positions: 1
  • Site: Fellowship training will take place at Victoria Hospital. Additional cardiac anesthesia training will take place at Sick Kids Hospital, Toronto.
  • Service commitment: 100 days per year. This service commitment will be in pediatric anesthesia.

As a major referral centre for a population residing in a large geographical area including western Ontario, eastern Saskatchewan and Nunavut, the Children’s’ Hospital administers over 8000 anesthetics cases each year. Apart from neurosurgical, craniofacial,
thoracic, neonatal, major orthopedic and urologic, general, ENT, and plastic surgical procedures we also provide anesthetic care in ten off-site locations including neurosurgery with intra-operative MRI scanning (iMRIs). Regional anesthesia is a major component of anesthesia for children in Winnipeg.

The fellowship is flexible to accomodate the training needs of the fellow and can include up to an 8 week block of pediatric cardiac anesthesia block +/- a 4-week PICU rotation, subject to availability. Involvement in research is also an option, either in a new project or an ongoing one. Winnipeg may well be the coldest metropolis around, but its multiculturism and the prairie hospitality shines through.

How many Fellowships each year: Currently we offer 6 – 12 months paediatric anaesthesia fellowships, with a maximum of two fellows at any time, although we also offer shorter placements and tailor the program according to the needs of the fellow.

At Red Cross War Memorial Children’s Hospital we offer subspecialist training in Paediatric Anaesthesia in all specialties including cardiac, liver transplantation, burns, neurosurgery, general surgery, orthopaedics, ophthalmology, ENT, urology, plastics and paediatric sedation.

We offer supernumerary fellowships in paediatric anaesthesia from 6 months to 1 year in duration.  It is also possible to incorporate a period of time on the Paediatric Intensive Care Unit.  Due to the constraints of the health care system in South Africa these are voluntary positions and do not attract a salary.

These posts aim to provide training to an advanced level in paediatric anaesthesia and they are aimed at trainees who are near the end of their specialty training in anaesthesia, or consultants who wish to expand their paediatric anaesthesia skills.

Our department is extremely busy, and performs on average 9,000 anaesthetics annually and our supernumerary registrars should expect to perform approximately 600 anaesthetics during the year (see example logbook).

Initially our fellows should expect a high level of direct supervision of their work, especially in the first few weeks of their time at Red Cross War Memorial Children’s Hospital.  After performing a satisfactory probation period the supernumerary fellows will then perform on-calls at a registrar level.  With time and with appropriate supervision by consultants, fellows will be allowed to assume the role of lead clinician for elective lists and emergency work.  Dependent on level of experience some fellows may be able to subsequently perform junior consultant calls.  We also offer shorter placements and tailor the program according to the needs of the individual fellow.

There are many educational opportunities on offer for all trainees. We have weekly UCT Department of Anaesthesia academic meetings, twice weekly paediatric anaesthesia protected teaching sessions, and the opportunity to attend the active teaching program throughout the Department of Anaesthesia.

Our fellows are expected to present at the Tuesday morning teaching tutorials at least once during a 6 month period.  In addition, there is the opportunity to do departmentally funded courses including; MEPA, PLS, FATE and a TOE course.  We also encourage the fellows to complete a small research or audit project during their time here.

Assessment is both continuous and through formalised feedback and appraisals are every 3 months.

Registration with the Health Professions Council of South Africa (HPCSA) is required before commencement of the supernumerary fellowship.  This usually takes approximately 9 months.

Located six blocks from the Eiffel tower in the heart of Paris, Necker Enfants Malades is the oldest dedicated paediatric hospital in the world and it continues this spirit of innovation to this day. The hospital is a national tertiary referral center for all medical and surgical paediatric specialties. With 20 dedicated general paediatric and 3 cardiac theatres we provide >20,000 anesthetics per annum across all specialties including cardiac, neurosurgery and transplant surgery (liver, bowel, renal). In addition, NEM is the paediatric trauma center for the Paris Ile-de-France region, with paediatric anaesthetists serving as trauma team leaders. Fellow can rotate between the different units if they wish to spend more than one year in the department. 

The department runs weekly teaching and simulator sessions in which fellows participate.Research and QI projects are overseen by Professor Gilles Orliaguet and fellows have the opportunity to participate in ongoing research as well as being encouraged to initiate new projects.

All fellowships are post CCT positions open to EU candidates holding a CCT in anaesthesia. Applicants should demonstrate an interest in paediatric anaesthesia 

All successful candidates are required to register with the Conseil de l’Ordre des Médecins de Paris (French IMC/GMC equivalent) – the department will provide help with this process. An intensive course in French is highly recommended aiming for a B2 level. (B2 language certificate is mandatory for Non EU candidates)
